Overview

SCAN offers all benefit eligible employees residing in California the choice between three HMO plans with Aetna. All benefit eligible employees have access to a High-Deductible Health Plan (HDHP) with Aetna that provides coverage nationwide. Employees outside California also have access to an EPO plan with Aetna.

For those residing in California, please take note of the different network for each HMO plan and ensure that your preferred providers and hospitals are within the plan network. Services rendered outside of the network will not be covered and will be at your own expense (emergencies excluded).

Aetna Whole Health HMO

Southern California employees only

This network provides coverage for a small network of providers and medical groups in Southern California only. During enrollment, you will be required to select the primary care provider (PCP) for you and any enrolled dependents. 

Aetna Value HMO

California employees only

This network provides coverage to a limited network of providers and medical groups throughout California. During enrollment, you will be required to select the primary care provider (PCP) for you and any enrolled dependents.

California employees only

This network provides coverage to a broad network of providers and medical groups throughout California. During enrollment, you will be required to select the primary care provider (PCP) for you and any enrolled dependents.

Aetna Open Access Managed Choice HDHP

Offered in all states

This network provides access to a broad network of providers and medical groups throughout the United States. The HDHP plan does provide limited reimbursement for services received out-of-network. Selection of a primary care provider (PCP) is not required. 

AETNA 
EPO

Non-California employees only

This network provides access to a more limited network of providers and medical groups than the HDHP outside California. Visit any doctor in the Aetna EPO network without a referral. There are no out-of-network benefits on this plan.

PCP Designation

When you complete your enrollment, you must enter your primary care provider (PCP) identification number if you enroll in one of the HMOs. Each enrolled family member may designate their own PCP. Your PCP ID can be found directly on Aetna’s website using the instructions below. If you do not designate a PCP during your enrollment process, Aetna will auto-assign one to you. If you wish to change your PCP after you have completed your enrollment, you will need to update your PCP ID directly with Aetna by calling Aetna member services. 

  • Go to https://www.aetnaresource.com/m/SCAN

  • Once you find your desired physician, you will see the Primary Care ID listed on their profile.

  • Enter the 6-digit Primary Care ID during your enrollment. If your Primary Care ID is only 5 digits, please add a leading “0”. 

HMO vs HDHP VS EPO: What to Know

If selecting one of the HMO plan options, all care must be rendered within your primary medical group. All care is funneled through your primary care provider (PCP) and your PCP must provide a referral to see a specialist. Care received outside of your medical group will not be covered and will be at your own expense.

If you select the HDHP, you have the flexibility to self-refer to any provider within the network, no PCP referral required. Coverage is available out-of-network, however, benefits are significantly reduced. 

If you select the EPO, you have the flexibility to self-refer to any provider within the network, no PCP referral required. Care received outside of the network will not be covered and will be at your own expense.
